This is a San Driver Model PDT-A13-391
I believe it has never been used but I have no way to verify that so I'm offering it as used with a corresponding price.
I looked for signs of wear, perhaps where the retaining screws would have left marks on the mounting flange and found none. If you supersize the images you can see the mounting flange and overall condition quite clearly.

This is a used part. I purchased the salvage rights to a stock room of a plant that was shut down. This unit was on the shelf so I assume it was ready to use.
